Please complete the New Customer Form and bring it to the pharmacy on your first visit. Completed New Customer Forms can also be submitted by faxing it to Lakeview Pharmacy at (262) 632-6777.

Also, please review Lakeview Pharmacy's Notice of Privacy Practices document. After reviewing the document please print it out and sign where indicated, then bring it with you on your first visit to Lakeview Pharmacy.
