Let Us Take Care of You

All day, every day, you care for others. It’s our turn to take care of you. Since 1985, Lakeview’s long term care pharmacy continues to build strong relationships with community-based residential facilities and provides them with the essential resources that help them remain compliant with the state as well as meet their patients’ personal care needs.

We’re more than a pharmacy. We’re a team of experienced, licensed nurses and pharmacists that help caregivers do what they do best: care for others. Like you, we’re in it for the long haul.

Medication Review

Annual Medication Review for Your Residents

It’s your passion to take care of your residents. It’s our passion to help you. Lakeview Pharmacy offers both an on and off-site medication review for the residents of the long term care facilities we serve. We recommend annual medication reviews for each resident to ensure the best possible care. After each review, we communicate with their physician about any possible changes to medications or any other recommendations that may improve the overall health of a patient.

Medication profiles are checked for:

  • Appropriate Dosage

  • Age

  • Disease State

  • Dose Range

  • Drug Interactions – We review medication lists and patient histories for potential drugs interfering with other medications

  • Poly Pharmacy – Reduces the number of prescriptions when combination drugs are available

  • Lab Results

Contact us today or call 262-632-8215 to schedule your medication reviews with us.

Site Visits

Annual Visits Help With State Compliance

You’re busy caring for residents. Our job is to keep you going. Aware of the ever-changing state regulations, we help your long term care facility remain compliant with the state and ahead of the game. We recommend annual site visits to your facility to review for compliance with DHS 83 as it pertains to Medication Administration.

During our site visits, our pharmacist will come to you and review the processes for the medication system, structure of your facility, competency of staff, and medication administration accountability. We offer recommendations to correct any issues we find.

Contact us today or call 262-632-8215 and set up your site visit today!
